This most commonly happens when the mechanical clearances around the sash are not equal, preventing a perfect half circle arch from being drawn.

3. The solution
Adjust the mechanical clearances so the top clearance is the same as the left and right clearances. Change the defaults and templates to prevent this from recurring.
